It's a reverso. Works as a self braking belay device. You need two screw-gates.

edit: I'm sorry, you are correct. something's very wrong. But I'm not sure if it disable's the selfbreaking effect. someone with a clear head and knowledge should reply on this

i do not have a reverso, but a friend of mine does, and as far a i know, the belay devise should be hanging from the hole! next to the hole the ropes go in. and not like it does in the photo. the hole is nearly not visible in the photo.
it is indeed used wrongly. In order to use it in self-lock mode you need a second carabiner. Like here http://www.supertopo.com/photos/1/7/132259_10705_XL.jpg The way it's used in the photo makes it veryhard to hold a fall of the seconding climber, especially if there is only little rope drag and he/she is heavy.
